car vibration

i have a 1992 fiat126bis the the engine vibrates when in gear and under load, the engine mounts are all new, new clutch and the flywheel is straight . someone has mentioned balance shaft?. the vehicle idle all ok, the vibration starts when you put it in gear and drive it
